What are the defects?

The thread sleeve that the backrest knob screws into can become loose if over tightened, this could cause the backrest to come off.

What are the hazards?

If the backrest comes off while someone is sitting on the Rollator they could fall backwards, resulting in an injury.

Yeah a bad one. If you are already disabled enough to need to use one and it fails the results can be life threatening eg broken hips and older people do not mix well, osteoporosis and a then broken back or neck…Just bad.
