It may help others to respond if there are some further details concerning the proposed relocation.

Have you contacted the Commonwealth Govt Disability Support Services for their advice/support?

Assume you are inquiring specific to moving an individual and not their personal property.
We moved our mum (wheel chair required) from Newcastle to SE Qld last year pre Covid. She was in respite care in NSW and approved to go into aged care due to a loss of mobility and physical independence. She is on a full single pension. Support for the aged has different criteria to those eligible for the NDIS.

We were advised by aged care services in NSW we would need to organise the move ourselves. We were also advised that we could arrange for the ambulance services in each state to assist, however it would not be free. It may be advisable to discuss your needs with your state ambulance service. We obtained a medical assessment to support flying. The airline was very accommodating. There are also taxi transport services that can accommodate wheel chair and other needs, if flying is an option.

I found your post useful, I am trying to transport my Mum from one aged care facility to another (Northern NSW to Northern Victoria), when I search online to do this it is like a minefield. My mum had a stroke and is paralysed down one side. My mum is on a full pension too. So you flew your Mum? Can you please advise who you used and nay contacts you might have? Thanks

Welcome to the community @Celi.

We self managed the move from rehab attached to the hospital in NSW to a regional aged care facility in SE QLD.

Whether that is practical in your instance the following may assist.
Our mum (actually my mum-in-law) was fully aware, but limited in mobility requiring a wheel chair for any distance and a walker with one/two in support for very short movements. There were two of us accompanying her all the way. We had a vehicle at both ends of the flight. Approx 1 hour road time at each end.

We discussed the transport and journey in advanced with the aged care specialist in rehab. As previously mentioned we obtained a medical statement from the Doctor responsible for her to confirm her fitness to fly and be in an aircraft.

We’ve arranged or been aware of flights for several of our older family members. If you expect it is practical to fly and manage yourself pre-Covid our experience of Qantas, Jetstar and Virgin was all positive. I’d not recommend any in todays environment. We flew our mum business (the old Virgin as I had points in hand). This streamlined some of the terminal procedures as well as making the onboard accommodation easier to manage.

It may pay to talk with REX if their services are more convenient to your two end points.

We really needed two at both ends to handle the hire cars, luggage, trolley, wheelchair etc.

Re NSW health and the Commonwealth Aged Care Services, we found them of little practical assistance in how to get from one place to the other. It’s possibly a result of moving between states. We would consider that a not uncommon option, however it appeared from the staff reactions we were abnormal. It seemed unusual for those entering aged care to be placed outside the immediate health district/zone.

Apologies if you are looking for an ambulance transport level service. We were able to manage without the added expense.
